Blue Cross Blue Shield of North Dakota (BCBSND) offers this free Health Risk Assessment (HRA) for the purpose of educating individuals about their health. Completion of the HRA is voluntary and all information provided on the HRA will be strictly protected by BCBSND and will not be used by BCBSND for any purpose other than generating the online personal wellness report at the end of the HRA. The HRA is not intended for use by employers as part of a wellness program or any other employee incentive program. Employers should consult a qualified legal professional regarding potentially applicable local, state and federal laws, including the Genetic Information Nondiscrimination Act of 2008 and the Employee Retirement Income Security Act of 1974, before using this HRA with employees.
