MyHealthCenter Only Enrollment Process
Follow the steps below to use MyHealthCenter. You will be eligible for wellness rewards and incentives once your health risk assessment is finished.
1. Go to: MyHealthCenter
2. Click on "Not a Member"
3. Enter membership information
4. Complete online authorization annually
5. Enter e-mail address, mailing address and phone number
6. Fill out your profile
7. Create a username and password
8. Click on "click here to begin your health assessment" (annual process)
