Enrollment Process
Follow the steps below to use MyHealthCenter and Health Club Credit program. You will be eligible for wellness rewards and incentives once your health risk assessment is finished.
Part 1 - MyHealthCenter:
1. Go to: MyHealthCenter
2. Click on "Not a Member"
3. Enter membership information
4. Complete online authorization annually
5. Enter e-mail address, mailing address and phone number
6. Fill out your profile
7. Create a username and password
8. Complete your health risk assessment annually
Part 2 - Health Club Credit:
1. Bring your BCBSND ID card to your health club
2. At your participating health club, ask to enroll in the Health Club Credit program
